This Privacy Policy applies to the DEXTENZA360™ PORTAL (the "Client Portal") (found at owned by Ocular Therapeutix, Inc. ("Ocular Therapeutix") and operated by its agent EVERSANA, a covered entity under HIPAA. For the purposes of this Privacy Policy, Ocular Therapeutix, the Client Portal operators and administrators, and their respective agents, affiliates, and partners are referred to as the "MPDP Parties" and "we," "us," or "our." This Privacy Policy describes how the MPDP Parties collect and use the personal information you provide on the Client Portal. It also describes the choices available to you regarding our use of your personal information and how you can access and update this information. The use of information collected through the Client Portal shall be limited to the purpose of providing the services and information for which you have engaged us, as more specifically detailed below.
You acknowledge that Ocular Therapeutix is the owner of the Client Portal but does not operate it. Ocular Therapeutix does not have access to any protected health information ("PHI") provided in connection with the Client Portal at any time for any purpose. All functions involving PHI are performed and managed under the direct control of EVERSANA. For the sake of clarity, PHI is the protected health information of patients in connection with the DEXTENZA360 program. The MPDP Parties cannot and do not assume any responsibility for your use or misuse of PHI or other information transmitted, monitored, stored, or received while using the site.


The MPDP Parties may collect personal information from you such as your name, NPI number, practice name, position, address, phone number, username, enrollment data, and email when you actively provide the information, such as when you register on the Client Portal or otherwise communicate with the Client Portal.


We do not sell or rent your personal information to anyone. Any information collected by the MPDP Parties is password protected for privacy and security and will be treated as confidential. We use customary technology to protect the security of information, but we cannot ensure or warrant the security of any information you transmit to us. We disclose collected personal information only to our contractors and affiliated organizations that (i) need to know that information in order to provide services available in connection with the Client Portal, and (ii) have agreed not to disclose it to others. By using the Client Portal, you consent to the transfer of such information to them.

Use of Personal Information

The MPDP Parties may use your personal information for customer service, to provide you with information that you may request, to customize your experience on the Client Portal, and to process your transactions on the Client Portal or otherwise facilitate services related to the DEXTENZA360 program, including sending you information, news, updates, and other opportunities. Services related to the Client Portal include, but are not limited to, registering you for access to the Client Portal, providing email and/or text alerts (if you opt-in to receive them) regarding new information arriving in your Client Portal account, and providing email notifications should a critical event occur that impacts operations of the DEXTENZA360 program. We may also use your information for internal purposes and to illustrate trends and patterns. We may use collected nonpersonal information in the aggregate to study statistical results and produce summary reports.
We may share your personal information with third parties, but only in the ways that are described in this privacy statement, and otherwise as needed to support the permitted uses described herein.

Information for Patients Regarding the DEXTENZA360 PORTAL

Ocular Therapeutix has no direct relationship with the individual patients whose personal data is processed through this Client Portal. If you are a patient of a healthcare provider utilizing this Client Portal and you have questions or concerns about this Client Portal, please contact your healthcare provider directly.


We may disclose your personal information as required by law, such as to comply with a subpoena, bankruptcy proceedings, or similar legal process when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request. If any MPDP Party is involved in a merger, acquisition, or sale of all or a portion of its assets in a manner that affects this Privacy Policy, you will be notified via email and/or a prominent notice on the Client Portal, of any change in ownership, uses of your personal information, and choices you may have regarding your personal information.

User Access and Choice

If your personal information changes, or if you no longer desire our service, you may ask to have it amended or your account deactivated by contacting the DEXTENZA360 program at 800-339-8369 or We will respond to your request within a reasonable timeframe.

Data Retention

We will retain your information for as long as your account is active, or as needed to provide you services. We will retain and use your information as may be necessary to comply with our legal obligations and with HIPAA regulations, to resolve disputes, and to enforce our agreements.


The security of your personal information is important to us. We follow generally accepted standards to protect the personal information submitted to us, both during transmission and once it is received. Log Files

As is true of most websites, the DEXTENZA360 PORTAL gathers certain information automatically and stores it in log files. This information may include Internet protocol (IP) addresses, browser type, Internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and/or clickstream data. We do not link this automatically collected data to other information we collect about you.

Tracking Technologies; Cookies

The MPDP Parties use analytics and technologies such as cookies, beacons, tags, and scripts to analyze trends, administer the Client Portal, track users’ movements around the Client Portal, and gather demographic information about our user base as a whole. We may receive reports based on the use of these technologies by these companies on an individual and aggregated basis.
We use cookies to enable your device to remember your preferred settings and for authentication. Users can control the use of cookies at the individual browser level. If you reject cookies, you may still use the Client Portal, but your ability to use some features or areas of the Client Portal may be limited.

Examples of Client Portal cookies and their uses:

  • Session cookies: These are provided by the framework technology we use for the Client Portal web application, specifically for use during sign in.
  • First-party cookies: Used for tracking when a User has selected the “remember me” check box to avoid having to answer security questions on every login.
  • Third-party cookies: Anti-forgery tokens used in securing the Client Portal web application, provided by Microsoft.
  • Third-party analytical cookies: Used for Google Analytics of Client Portal usage trends.
  • Persistent cookies: Also used for tracking when a User has selected the “remember me” check box to avoid having to answer security questions on every login.

Third parties with whom we partner to provide certain features on the Client Portal or to display advertising based upon your web browsing activity use Local Storage, also referred to as HTML5, to collect and store information. Various browsers may offer their own management tools for removing HTML5.

If you submit personal information to any of those websites, your information is governed by their privacy statements.
